Submitting your nomination:

• Nominators are responsible for submitting the complete Nomination package consisting of:
  
  
  • Signed Nomination forms (see above tab or click here)
  • Signed Nomination letter 
  • Three additional signed letters of reference
  • Curriculum Vitae
  • Proof of age. Nominees must be under 50 years of age by January 15, 2010. Copy of birth certificate or passport page is acceptable.
    
    
• We require all letters to be signed originals.
  
  
• We do not accept nominations by fax or email.
  
  
• Complete Nomination packages must be received by the Prize office by January 15, 2010.
  
  
• Nominations missing any of these components by the deadline will not move forward for review.
